Living Smart Diabetes ClassAs a member of the Living Smart Diabetes Self Management Program, you have access to a team of experts who are here to assist you with your care.

Patients are introduced to all of the resources necessary to treat and manage their condition in the most effective and pleasant manner.

Diabetes Education Sessions / Workshops

Individual Session

A one-on-one sit-down with our Educator to answer your questions on diabetes basics, nutrition, pump training or other diabetes related topic.

An opportunity to have the Educator assist you with planning of your meals and guidance with desired weight loss or weight gain. Goal setting and how to best achieve set goals is also discussed.

Basics Class

A four (4) hour, Saturday, instructional workshop that will allow you to learn from and participate in the following discussion:

  • How to describe the different types of diabetes
  • The impact of physical activity and stress on glucose control
  • A review of blood sugar goals, proper times to test blood sugar and recommended targets
  • The complications related to diabetes and how to reduce your risks
  • How to set and increase success of achieving set behavior goals
  • A review of approved medications for treating diabetes including common side effects and proper dosing
  • A list of some local organizations providing diabetes related assistance

Nutrition Class

A four (4) hour, Saturday, instructional workshop that will allow you to learn from and participate in the following discussion:

Basic Nutrition on Carbohydrate Counting, Glycemic index, Fiber, Water, Sugar Alcohols, Sugar Substitutes, Protein and Fat facts

Meal Planning review of Label reading, Portion sizes, Meal replacements, Alcohol and Eating Out, Guidelines for low blood sugar and sick day and a session on planning a sample menu.

Annual Refresher

A two (2) hour, Saturday, instructional workshop that will allow you to learn from and participate in the following discussion:

  • A review of blood sugar goals, proper times to test blood sugar and recommended targets
  • A review of diabetes related complications including low and high blood sugars, how to prevent, treat and reduce risks
  • Healthy eating, monitoring and physical activity goals and how to set and increase success of achieving goals
  • A review of carbohydrate counting, label reading and portion sizes
  • An update on the latest approved medications and ongoing clinical trials
  • A discussion on stress management and coping with diabetes

Diabetes Prevention Program

Hanson Diabetes Center is taking registration now for its Lifestyle change program which will start January 2018. One of the goals of the program is to seek The Centers for Disease Control and Prevention (CDC) recognition.

To ensure high quality, CDC recognizes lifestyle change programs that meet certain standards and show they can achieve results. These standards include following an approved curriculum, facilitation by a trained lifestyle coach, and submitting data each year to show that the program is having an impact.

A CDC-recognized lifestyle change program is a structured program—in person or online—developed specifically to prevent type 2 diabetes. It is designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes.

A trained lifestyle coach leads the program to help you change certain aspects of your lifestyle, like eating healthier, reducing stress, and getting more physical activity. The program also includes group support from others who share your goals and struggles. This lifestyle change program is not a fad diet or an exercise class. And it’s not a quick fix.

It’s a year-long program focused on long-term changes and lasting results.

A year might sound like a long commitment, but learning new habits, gaining new skills, and building confidence takes time. As you begin to eat better and become more active, you’ll notice changes in how you feel, and maybe even in how you look.

Other Workshops

Intensive Insulin Management ( 2 hrs)
For patients on multiple insulin injections. A workshop on carbohydrate counting and pattern management to better stabilize blood sugar control.
Insulin Pump Introduction
Periodic workshops also given on insulin pump introduction and training.
